This program is conducted on-line, initiated by a completed extensive Life Change Questionnaire mailed to the Know Your Options office in California. The response to this questionnaire is evaluated to determine an individualized Self-Help Support Program. This program, with the daily protocol, including guidelines and product for 30 days is shipped directly to patients. This Self-Help Support Program requires patients to communicate by email and telephone calls as well as report-back forms by fax, email, or mail. This feed-back and monitoring is used to make adjustments for subsequent 30 day cycles.
